Our goal is for your experience with our office to be unlike any that you have ever experienced. We respect your time and it is because of this that we provide our new patient forms online. By taking 10-15 minutes to complete our online information in the comfort of your home, our office can verify insurance benefits, review your health history and better prepare for your first visit. You will need your dental insurance card (for our patients with coverage) and any prescription medications you are taking. Please fill all sections of the forms at one sitting. If necessary you may close out of the session and log back in

Our initial examinations are by nature comprehensive and therefore require the same comprehensive radiographs (x-rays). To offer you the latest in technology, our practice uses digital radiography, which uses up to 80% less radiation than traditional x-rays and allows us to see things that may otherwise go undetected.
Please arrange for any films from your previous dentist to be in our office by the time of your initial appointment. We will supplement your x-rays with ours. If we do not have your films at that time, we will take new films using digital radiography. Please take the time to complete the online registration forms and submit them 48 hours (2 business days) prior to your appointment. This will aid us in serving you better and using your appointment time more efficiently. As insurance is a contract between you and your insurance company, failure to provide us with the complete information will prohibit us from filing forms for you. |
